Managing Your Care
Helping you manage your healthcare is our mission at the skilled nursing facility Dove Healthcare - South Eau Claire.
The more you understand about your health and are an active participant in your care, the more you will retain your independence. Licensed staff provides individualized teaching and tools in the areas of pain, medication, and diabetic management; ostomy and wound care; ambulation and transfer techniques; and oxygen therapy. Support is also available to help with lifestyle changes and information and referral assistance to connect with community resources.
Your care plan will be flexible and tailored to meet your needs.
Your care plan directs the caregivers on how to care for you by detailing your strengths, weaknesses, preferences, routines, and personal history. The care plan is developed by an interdisciplinary care team through on-going communication with you and your family. It includes activities of daily living, cognition, nutrition, communication, activities, safety, discharge planning, and psycho-social needs.
During your stay at Dove Healthcare - South Eau Claire, your family and you will meet regularly with the care team to review your care. All aspects of your care are discussed at a "care conference" meeting, including your nursing, therapeutic, nutritional, recreational, and spiritual needs. Your discharge plans are also discussed at these meetings.
The role of your RN Case Manager
Our RN Case Managers enjoy both the gratification of patient care and the rewarding challenge of leadership. From the moment a patient is admitted to Dove Healthcare - South Eau Claire, to the day they go home, a RN Case Manager plays a fundamental role in making sure each patient receives person-centered care. RN Case Managers must evaluate the patient's health status; facilitate the preparation of an individualized plan of care; and manage the implementation of nursing services in order to meet the patient's healthcare needs.
